5. Email Opt-Out

You can anytime stop receiving any further communications from Surveymonster, by going to the Unsubscribe page which is clearly visible on the footer of all the pages of the site and in all the emails you receive from surveymonster.net or by directly going to https://surveymonster.net/unsubscribe. You will also get unsubscribed from surveymonster if you hit the SPAM button on your email. No further emails are ever sent to you, once you get unsubscribed.

6. How can you update or correct your Personally Identifiable Information?

We believe you should have the ability to access and edit the personal information that you have provided to us. During the registered membership process you can review, change, or delete the information that you have submitted. You may change any of your personal information in your account online at any time. We encourage you to promptly update your information if it changes.

7. What security precautions are in place to protect against the loss, misuse, or alteration of your information?

In order to most efficiently serve you, established third party banking, processing agents and distribution institutions handle credit card transactions and order fulfillment. They receive the information needed to verify and authorize your credit card or other payment information and to process and ship your order.

This site has security measures in place to protect the loss, misuse, and alteration of the information under our control. The data is backed up on regular basis on site and a copy is kept off site. SurveyMonster.net reviews its security measures on a regular basis by utilizing the various technologies being developed and updating password access as necessary.

8. A special note about children's privacy

You must be at least 18 years old to have our permission to use this site. Our policy is that we do not knowingly collect, use or disclose Personally Identifiable Information about visitors under 18 years of age. If you are the parent or guardian of a minor under 18 years of age and believe that they have disclosed personally identifiable information to us, please contact us by email so that we may delete the minor's information.

13. You Have the Right Not to Have Your Personal Information Sold

You have the right to request that we do not sell any of your personal information. Personal information for this section means a natural person's first name or first initial and last name in combination with any one or more of the following data elements when the name and data elements are not encrypted: social security number, driver's license number, driver authorization card number, or identification card number. Account number, credit card number, or debit card number, in combination with any required security code, access code, or password that would permit access to the person's financial account. A medical identification number or a health insurance identification number. A username, unique identifier, or electronic mail address in combination with a password, access code, or security question and answer that would permit access to an online account. Please include enough personal information so that we can reasonably verify your identification.

We collect information that identifies, relates to, describes, references, is capable of being associated with, or could reasonably be linked, directly or indirectly, with a particular CA User or device ("personal information"). In particular, we have collected the following categories of personal information from CA Users within the last twelve (12) months:

Personal information does not include:

  • Publicly available information from government records.
  • De-identified or aggregated CA User information.
  • Information excluded from the CCPA's scope, such as:
    • health or medical information covered by the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and the California Confidentiality of Medical Information Act (CMIA) or clinical trial data; and
    • personal information covered by certain sector-specific privacy laws, including the Fair Credit Reporting Act (FRCA), the Gramm-Leach-Bliley Act (GLBA) or California Financial Information Privacy Act (FIPA), and the Driver's Privacy Protection Act of 1994.

We obtain the categories of personal information listed above (including (a) full name; (b) e-mail address; (c) mailing address; (c) daytime and/or cellular telephone numbers; and (d) date of birth) from the following categories of sources (with the specific categories of personal information indicated in parenthesis):

  • Directly from our CA Users. For example, from online registration forms that our CA Users submit to us in connection with our membership services, our sweepstakes and promotions and the other products and/or services that we offer by and through the Site. (Category A, B and C)
  • Indirectly from our CA Users. For example, through information we collect from our CA Users in the course of providing our products and/or services to them. (Category A, B, C and F)
  • Directly and indirectly from activity on the Site. This includes the type of browser that you use (e.g., Safari, Chrome, Internet Explorer), your IP address, the type of operating system that you use (e.g., Windows or iOS) and the domain name of your Internet Service Provider. We may use or disclose the personal information that we collect (including (a) full name; (b) e-mail address; (c) mailing address; (c) daytime and/or cellular telephone numbers; and (d) date of birth) for one or more of the following business purposes (with the specific categories of personal information indicated in parenthesis):

  • To fulfill or meet the reason for which the information is provided. For example, if you provide us with personal information in connection with your registration for our sweepstakes and promotions and/or our other products and/or services, we will use that information to process your registration. Sharing Personal Information
